This is mostly likely due to either a permissions problem on the storage location of your documents, or the DOCUMENT_DIRECTORY configuration option has changed in your config.inc.php file. The DOCUMENT_DIRECTORY configuration option defines where you store your documents. The directory and all files/folders in this directory should be readable and writeable by the web server.
define("DOCUMENT_DIRECTORY","/storage/document_area");
If you test this under Microsoft Windows, remove the first "/" like this:
define("DOCUMENT_DIRECTORY","storage/document_area");
